Position Summary

The Admissions Coordinator – Home Health is responsible for processing patient referrals from intake to admission to home care.


Principal Accountabilities

  • Verify patient’s insurance coverage per agency policy
  • Communicate to staff any referral, admission, death or discharge
  • Responsible for initial data entry of referral
  • Identify and prioritize potential referral partners
  • Develop and maintain a close working relationship with all of the team disciplines
  • Maintain a system for patient discharges
  • Obtain orders and documentation needed for admissions
  • Provide home health informationals to patients and families as needed
  • Participate in home health programs; providing staff orientation, training, and in-service

Qualifications

  • Effective organizational, interpersonal and communication skills
  • Must have reliable transportation
  • High sense of integrity
  • History of effective communication with physicians, nurses and other healthcare personnel.
  • Knowledge of Microsoft Office programs

Education and/or Experience

High School Diploma or General Education Degree or two to three years related experience and/or training; or equivalent combination of education and experience. Bachelor’s Degree preferred. Experience in insurance and Medicare/Medicaid billing and reimbursement preferred.
