As a Registered Nurse with the Goals of Care Program, you will serve as a consultant for partner organizations to complete Goals of Care conversations and documentation with the organization’s members. You will support the member’s frontline provider by devoting time to family meetings dedicated to the discussion of treatment goals. As the RN, you’ll facilitate the completion of advanced care planning documents while applying the rules and regulations of state and federal regulatory agencies and other certified agencies. The RN serves as clinical and educational role model and mentor; provides support in collaboration with the primary care team; and ensures compliance with practice standards and codes.


PRINCIPAL ACCOUNTABILITIES

  • Supports agency and corporate policies, goals, and objectives.
  • Attends and participates in staff meetings, educational programs and community events, as requested.
  • Promotes palliative care and hospice philosophy; communicates identified needs and potential solutions to leadership.
  • Performs Goals of Care conversations with members and/or families.
  • Educate patient and family, and provide emotional support, to enhance understanding of disease processes and plan of care, including discussion of treatment options to ensure informed decision making.
  • Empower patients to take control of their health care by encouraging appropriate goal-setting and discussions about advance care planning/completion of advance directives.
  • Partner with healthcare team to manage transitions of care to hospice or palliative care when appropriate.  
  • Assists patients and families in identification of goals of treatment and ongoing plans of care and communicates these goals to the healthcare team.
  • Help to triage new consults to prioritize case load.
  • Responsible for the preparation and maintenance of accurate patient records, charts, and documents.

  • Ability to read, write and speak professionally in the English language, including documentation of regulations.
  • Effective presentation of information and response to questions from leaders, Colleagues, patients and family members, and the general public.
  • Knowledge of programs in Microsoft Office.
  • Possess strong organizational and interpersonal skills and have the ability to work a flexible schedule, to include some evenings and weekends.
  • Must be able to travel in the regional area of coverage.
  • Ability to maintain confidentiality of information, such as patient and Company files.



EDUCATION AND/OR EXPERIENCE

  • Licensed Registered Nurse able to practice in state of employment.
  • Minimum of 1 year experience in hospice and/or palliative care preferred.
  • Associates Degree in Nursing required, Bachelor’s Degree preferred.
