Position Summary

The Hospice Care Coordinator is responsible for receiving incoming calls to presents Hospice services and benefits to patients/family and coordinates the assessment and the admissions process with the local program. You'll be responsible for applying the rules and regulations of state and federal regulatory agencies and other certified agencies.

Regulatory Compliance Pillar of Success: 

  • Reads and incorporates into practice the policies and procedures for Compassus as related to the Hospice Care Coordinator position.
  • Articulates the framework for regulatory compliance from federal perspective and the application/ implications for the Hospice Care Coordinator.
  • Adheres to company expectations for the protection of confidential information.

  • Initiates initial conversation with patient and families and ensures a smooth transition to the program.
  • Provides attentive follow up with patients/family and programs to promote a seamlessly process for the patients and families.
  • Receives incoming calls and promotes care to hospice appropriate patients.
  • Maintains a positive relationship with the programs.
  • Ensures proper notifications of admission to programs are accurately documented.
  • Documents incoming calls, as directed.
  • Analyzes data to provide leadership.

Knowledge and Skills

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or Experience

Bachelor’s degree. Three to five years’ experience in inside sales. Hospice, Palliative, hospital and/or long-term care clinical experience highly preferred.

Mathematical Skills

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.  Ability to compute rate, ratio, and percentage.

Language Skills

Strong communication skills. Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or government regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information one on one and be able to respond to questions from colleagues, patients, families, and the general public.

Reasoning Ability

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or schedule form.

Computer Skills

To perform this job successfully, an individual should have knowledge of Microsoft Office software.

Other Qualifications

Possess strong leadership, organizational and interpersonal skills. Ability to work a flexible schedule, to include some evenings and weekends. Must be able to travel. Ability to work independently in the management of assigned responsibilities.  

Certifications, Licenses and Registrations

A valid driver’s license and auto liability insurance.

Other Skills and Abilities

Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy. Must be of high integrity including maintenance of confidential information including all patient records and Company files.
