Position Summary

The Quality Assurance Nurse assesses the quality of care provided to hospice patients, families, and staff. S/he monitors documentation, compiles quarterly reports, and works closely with the agency Director Clinical Services.

Principal Accountabilities

  • Communicates identified needs and potential solutions to supervisor.
  • Maintains charts to meet Medicare/Medicaid regulations.  
  • Keeps informed of current Medicare/Medicaid regulations.
  • Ensures audit of records at regular intervals to assess for compliance.
  • Reports deficiencies to appropriate staff for corrections and follows up to ensure completion.
  • Assists in test edit chart reviews, as requested by fiscal intermediary.
  • Assesses quality of documentation.  
  • Audits patient records in house.
  • Ensures that the plans of care are updated to reflect new orders, intervention, and instructions.
  • Reviews IDT documentation for accuracy and completeness.
  • Reports deficiencies to appropriate staff for corrections and follows up to ensure completion.
  • Submits quarterly report for Governing Board to ED.                 
  • Works in tandem with Corporate total quality care personnel, and assesses the quality of care and services provided by agency.
  • Ensures documentation reflects the quality of care and services provided.
  • Ensures that continuity of care is reflected in patient record.
  • Works with ED to ensure QAPI program is in compliance.
  • Leads QAPI meetings.
  • Assists ED/MCD with Plan of Correction following internal/external surveys/audits.
  • Facilitates and monitors PIPs.
  • Conducts focused audits and submits results to Executive Director.
  • Works with Director of Clinical Services in deficient areas and monitors effectiveness of resolution.

Qualifications

  • Communication, leadership, and interpersonal skills
  • Ability to tend to the geriatric and terminally ill
  • Proficiency in Microsoft Office Suite.
  • Ability to work a flexible schedule, to include some evenings and weekends
  • Ability to travel as required

Certifications, Licenses and Registrations

  • Must have a valid driver’s license and auto liability insurance.
  • Professional certification(s)/license(s): See Education/Experience: Specific State Requirements

Education and/or Experience

Registered nurse licensed in the state of employment. Knowledge and experience in quality improvement and/or utilization review. Hospice, Oncology, or Home Health and/or long-term care preferred.

State-specific Certifications:

  • Louisiana: Two years full time experience as a Registered Nurse or two years clinical in hospice.
