Position Summary

As a Triage Nurse with Compassus, you'll have the opportunity of providing triage nursing care to families across the country while having the comfort of working in an office setting. The Referral/Triage Center LPN functions as an extension of the Interdisciplinary Team (IDT) in order to provide routine and emergency assessment of patients, education to patients and caregivers, and implementation of telephonic nursing interventions which support the patient and family in maintaining their maximum level of comfort and coping. 


Principal Accountabilities

  • Explains hospice services and Medicare benefits to patients and families via telephonic interactions.
  • Submits appropriate documentation and paperwork at the completion of telephonic interactions.
  • Effectively communicates patient and family needs to IDT via documentation.
  • Works collaboratively in a team environment to promote the values of hospice and of Compassus.
  • If RN, provides supervision to LPN/LVN colleagues including clinical guidance and decision-making support.
  • Answers telephone promptly (within 2 rings) and responds with clinically and situationally appropriate interventions based on telephonic assessment.
  • Communicates with physicians or nurse practitioners as indicated regarding measures to alleviate symptoms, and provides instructions to patients/ families regarding expected outcomes and responses.
  • Utilizes pharmacological and non-pharmacological interventions based on the patient’s plan of care and available nursing procedures to achieve symptom control.
  • Implements on-site nursing visit by member of the local hospice team when appropriate.
  • Appropriately records interactions, interventions and other pertinent information; submits documentation timely.
  • Prevents unwanted/ unwarranted hospitalizations where possible and communicates effectively with local hospice team.
  • Communicates all deaths, adverse events, and symptom escalations to local hospice nurse for immediate on-site support where telephonic intervention is not appropriate.
  • Receives referrals and captures all necessary information to ensure local hospice nurse can begin in-person interaction for admission evaluation.
  • Reports complaints/ grievances to program for follow up and resolution.

Education and/or Experience

Registered Nurse with a current, unrestricted nursing license in state of employment. Must obtain licensure in all states served via triage center within 6 months of date of hire. Minimum two (2) years’ nursing practice or equivalent experience preferred. Hospice, Oncology, Home Health and/or long-term care experience preferred.


Experience with crisis management, clinical assessment and bedside nursing interventions is strongly preferred.  Experience with multiple documentation systems including EMR is preferred.


Licensed Practical/ Vocational Nurse with a current, unrestricted nursing license in state of employment. Must obtain licensure in all states served via triage center within 6 months of date of hire. Minimum two (2) years’ nursing practice or equivalent experience preferred. Hospice, Oncology, Home Health and/or long-term care experience preferred. Experience with multiple documentation systems including EMR is preferred.


State Specifics:

  • Louisiana: Two years full time experience as a Registered Nurse. Two years’ full-time clinical experience as a Licensed Practice Nurse in hospice shall be sufficient in place of clinical experience as a registered nurse.
  • Mississippi: 1 year full time experience as a Registered Nurse or 3 years full time continuous employment as a Licensed Practical Nurse immediately preceding Registered Nurse licensure

Mathematical Skills

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.  Ability to compute rate, ratio, and percentage.


Language Skills

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.  Strong written communication skills is required. 


Reasoning Ability

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or schedule form.


Computer Skills

To perform this job successfully, an individual should have knowledge of Microsoft Office software.


Certifications, Licenses and Registrations

CPR, current Michigan (MI) RN license and in good standing with the board.


Other Qualifications

Possess strong leadership, organizational and interpersonal skills.  Must be of high integrity including maintenance of confidential information including all personnel and patient records.


Other Skills and Abilities

Good Communication skills. Understanding and acceptance of hospice philosophy. Ability to understand, read, write and speak English.  Other language proficiency a plus.
