Position Summary

The Referral/Triage Center RN is responsible to the Manager of Customer Service Center to provide triage nursing care and to patients and families. Implements nursing interventions and coordinates with the local office following the Interdisciplinary Plan of Care. The RN adheres to the regulatory process required by the State and Federal guidelines and Company policy.


Principal Accountabilities

  • Answers telephone within 2 rings and documents all telephone calls.
  • Responds to all telephone calls within skill level and company policies and protocols.
  • Assess, via telephone, the condition of the patient. Instruct pharmaceutical and non-pharmaceutical interventions to manage symptoms and prevent hospitalizations when appropriate, depending on patient’s code status.
  • Utilizes relevant data to identify and resolve patient/caregiver complaints and concerns as appropriate.
  • Manages patient/caregiver calls and symptom management by assessing, implementing, evaluating and documenting the care provided as appropriate.
  • Supervises documentation of Referral/Triage LPN for appropriate interventions.
  • Ensures local office staff attend deaths of patients to assist and support the family (hospice).
  • Submits paperwork timely.
  • Participates in regularly scheduled team meetings to coordinate the care of the patient and family, exchange information and problem solve, and receive staff support and education. Makes referral to other disciplines as needed.
  • Participates in programs providing orientation, training, and in-service.
  •  Applies the Company policies and procedures to the rules and regulations of Federal and State regulatory agencies and other certifying agencies in providing care.
  • Assesses and evaluates patients’ level of care needs and requirements and directs local staff to provide visits as required.
  •  Receives information on referrals to hospice/homecare and enters information into the computer and ensures local office follow-up on assessing and admitting patients that are appropriate for hospice and/or homecare.
  •  Accepts other assignments deemed appropriate.
  •  Participates in agency quality improvement programs as requested.
  •  Provides direction to LPN as appropriate and documents supervision per Company policy.
  • Adheres to established Company policies.
  • Serves as an active member of the sales team.

Qualifications

  • Strong communication and interpersonal skills
  • Knowledge of Microsoft Office programs
  • Ability to manage patient care with specific knowledge and experience in bedside nursing, symptom management, crisis intervention and family intervention
  • Ability to work well with an interdisciplinary team
  • Reliable transportation

Education and/or Experience

Currently licensed as a Registered Nurse (RN) in state of work and in good standing with the Board. Obtain RN license in all states that are serviced by Compassus within six (6) months of hire. Minimum two (2) years of experience in nursing, surgical, oncology, homecare or hospice nursing preferred. 

Certifications, Licenses and Registrations

CPR, current Michigan (MI) RN license and in good standing with the board.
